For all YW and Adult Leaders attending Camp

All forms are PDF and can be edited online then printed. You can scan it or take a picture to upload during registration. Please remember to give a copy of the Church Permission & Medical Release form to your Ward Camp Director. Church Permission & Medical Release Form

Required for everyone attending Camp. Attach when registering here and also print out a copy and give to your Ward Camp Director.

Health Screening Form

Required for everyone attending Camp. To be done in your Ward YW by a qualified Health Supervisor within 24hrs before leaving to Camp.

Staff First Aid & CPR Training Statement of Compliance

Required for all leaders attending Camp. Vista Stake will offer training Saturday, April 23rd at 8AM.
